Liability Waiver & Photo/Video Release

I, the student and/or parent or legal guardian (if the student is under 18), understand that participation in the Bay Harbor Ballet Intensive involves physical activity that carries inherent risks of injury. I voluntarily assume all risks associated with participation, including but not limited to injury, illness, or loss, whether foreseeable or not.

In consideration of participation, I hereby release and hold harmless Ballet Coast to Coast and Bay Harbor Ballet Intensive, its directors, instructors, staff, volunteers, and host facilities from any and all claims or liability arising from participation in the program, including travel to and from program activities, except in cases of gross negligence or willful misconduct.

I certify that the student is physically able to participate and that I am responsible for maintaining medical insurance coverage. I authorize Ballet Coast to Coast to obtain emergency medical treatment if necessary and agree to be responsible for any related costs.

I also grant permission for Ballet Coast to Coast to photograph, film, or otherwise record the student during program activities and to use such images or recordings for promotional, educational, or archival purposes (including websites and social media) without compensation.

I have read and understand this waiver.
